Role
Summary:
As a Nurse Practitioner in a facility-based setting, you will:
  • Conduct clinical interventional and episodic direct care visits
  • Evaluate and treat chronic, co-morbid illnesses
  • Monitor changes in resident conditions
  • Collaborate with physicians for overall medical management
  • Perform Annual Wellness Visits and coordinate care with other healthcare disciplines

Qualifications:
  • Valid Physician Assistant or Nurse Practitioner license in Ohio
  • Current board certification in your field
  • Master's Degree in Physician Assistant Studies or Nursing
  • Strong leadership, interpersonal, and problem-solving skills preferred
  • Excellent written and oral communication skills
  • Punctuality and strong teamwork abilities

Estimated Salary: $20 to $28 per hour based on qualifications.
