Home Delivered Meal Assessor

Toledo, OH Toledo, OH Part-time Part-time $20 - $25 an hour $20 - $25 an hour The Area Office on Aging services a 10-county area in NW Ohio by coordinating and providing local services to older adults, their families, and people with disabilities.

Responsibilities:
Responsible for conducting in-person home-delivered meal client eligibility determinations, initial assessments, and re-assessments throughout Lucas County.
The assessor is also responsible for making necessary referrals to the PASSPORT program and other social service organizations based on client needs that are identified during assessments.
Will compile program information and statistics as requested.
1.
Assist with the coordination of services with clients, other program staff, and staff from other community agencies.
2.
Participates in departmental, staff, or other assigned meetings.
3.
Determine client eligibility for home-delivered meals.
4.
Determine the needs of individuals and coordinate services.
5.
Make referrals to the PASSPORT program or other social service programs, if the individual meets eligibility requirements and/or conduct PASSPORT Program assessments or assessments for other social service programs to expedite getting consumers on the best programs.
6.
Compiles program information and statistics as requested.
7.
Coordinates client referrals with the Lucas County meal provider.
8.
Ability to access and enter client data into the Wellsky Aging and Disability database.
9.
Other duties as assigned.
Knowledge, Skills, and Abilities:

Ability to interact professionally and maturely with a variety of people.

Able to work outside and drive in various weather conditions.
4.
Ability to collect, assemble, and disseminate information within established timeframes.

Team/goal oriented.

Excellent record-keeping and organization skills.

Strong written and oral communications 8.
Proficiency in Microsoft Office Suite and web-based applications.
Qualifications:
Clinical license in the State of Ohio.
Licensed Social Worker or Registered Nurse preferred.
Have a passion for working with older adults and can professionally interact with a variety of people.
Must have strong oral and written communication skills and excellent organization and record-keeping skills.
